The cost of the iPhone 4 is usually $499 for regular customers. 
We are doing an exclusive deal for $150 for the iPhone 4. 

If you are already with Vodafone we need to provide you with a new number as this is a deal for new customers. To sign up I have attached 2 forms.  Please print and fill in the first page of the personal connection form and sign it.  If you wish to pay the $150 by credit card please print the vfnz customer payment form and fill in your credit card details.  This is the best form of payment as I can have the phone sent to you as soon as it is ready.  If this is not suitable you can pay cash and I will arrange this when the phone is ready.  I will also need a copy of the front of your drivers license or your passport (the photo page).  Once you have all of this please scan and email it to me.
